The Food and Drug Administration (FDA) has officially approved Eli Lilly's latest Alzheimer’s medication, donanemab, which will be marketed under the brand name Kisunla. This approval marks a pivotal milestone in Alzheimer’s treatment, introducing a new therapeutic option for adults grappling with mild cognitive impairment or early-stage Alzheimer’s disease.

Kisunla functions as a monoclonal antibody infusion administered every four weeks, designed to target amyloid in the brain—a hallmark characteristic of Alzheimer’s. Eli Lilly has reported promising outcomes from clinical trials, indicating a modest deceleration in the decline of memory and cognitive abilities among patients.

Dr. Ronald Petersen, a distinguished neurologist at the Mayo Clinic, emphasized the significance of Kisunla’s approval, noting its role in expanding the therapeutic repertoire available for managing this debilitating disease. He highlighted the potential benefits of having multiple treatment options, offering clinicians and patients greater flexibility in Alzheimer’s care.

The FDA’s decision stemmed from a rigorous late-stage clinical trial involving 1,700 participants, where Kisunla exhibited a notable 35% reduction in Alzheimer’s progression over 18 months compared to a placebo. Assessment utilized the clinical dementia rating scale, evaluating key areas including memory, orientation, judgment, problem-solving abilities, community engagement, household management, hobbies, and personal care.

Findings from the trial demonstrated efficacy levels akin to those observed with a similar FDA-approved drug, Leqembi, introduced last summer.

Despite its therapeutic promise, Kisunla, like other drugs in its category, does carry potential risks, including brain swelling and bleeding. While most adverse events recorded during the trials were mild, the FDA documented three fatalities linked to the drug.

Eli Lilly anticipates that patients may discontinue Kisunla once amyloid levels have sufficiently diminished. However, uncertainty remains regarding the potential resurgence of amyloid and the need for potential treatment re-initiation.

Costing $32,000 for a year’s supply, Kisunla is expected to be covered by Medicare, aligning with the agency’s commitment to supporting newly approved Alzheimer’s therapies, contingent upon real-world data collection to ascertain treatment effectiveness.

Patient testimonials, such as that of Pat Bishara from Carmel, Indiana, underscore the potential benefits of Kisunla. Bishara participated in both the phase 3 trial and its extension phase, reporting minimal side effects and crediting the treatment for maintaining her cognitive function.
