States with abortion bans saw birth control prescriptions fall post-Dobbs, study finds

Following the Supreme Court's Dobbs decision, a recent study published in JAMA Network Open has illuminated a troubling trend: a decline in prescriptions for birth control pills and emergency contraceptives in states with the most stringent abortion laws. Spanning from March 2021 to October 2023, the study uncovered a 5.6% decrease in birth control pill prescriptions in states with strict abortion restrictions compared to more permissive states.

Additionally, it highlighted a stark 65% drop in emergency contraceptive prescriptions from July to October 2023 in states with severe abortion laws, contrasting sharply with states maintaining broader abortion access.

Senior author Dima Qato, director of the University of Southern California's Program on Medicines and Public Health, emphasized that the closures of abortion and family planning clinics in restrictive states significantly contributed to this decline. These clinics traditionally served as primary avenues for obtaining birth control prescriptions, severely limiting options for individuals seeking contraceptive care. Moreover, uncertainty surrounding the legality of emergency contraceptives in states with abortion bans exacerbated the plummeting usage rates, despite their legal status nationwide.

Julia Strasser, director of the Jacobs Institute of Women's Health at George Washington University, cautioned about the complexities of interpreting trends in emergency contraception use solely through pharmacy data. While acknowledging an initial uptick in oral contraception use immediately after the Dobbs decision, she underscored broader societal shifts away from hormonal birth control methods, adding layers of complexity to understanding contraceptive behavior.

Co-author Rebecca Myerson, assistant professor at the University of Wisconsin, expressed surprise at the study's findings, challenging assumptions that restricted abortion access might encourage more consistent contraceptive use. She noted an initial surge in emergency contraceptive fills post-Dobbs resembling stockpiling behavior, followed by a significant decline, pointing to genuine barriers or confusion surrounding legal access.

The Guttmacher Institute's 2022 report highlighted that within 100 days of the Dobbs decision, 26 out of 66 clinics providing abortion care in 15 newly restricted states had closed. This closure exacerbated access challenges, particularly for those relying on these clinics for contraceptive services. Conversely, Idaho's progressive policy allowing pharmacists to prescribe hormonal contraceptives without age restrictions saw an increase in emergency contraceptive use, underscoring the critical role of accessible healthcare services in contraceptive access.

These findings underscore broader implications for reproductive health policies and healthcare access in the United States. Persistent confusion around the legality and availability of emergency contraception in states with abortion bans necessitates clear policies safeguarding comprehensive healthcare access, including contraception.
